Event:  Since August 2018 the U.S. Centers for Disease Control and Prevention (CDC) has confirmed multiple cases of antibiotic-resistant infections in U.S. patients following surgery at the Grand View Hospital in Tijuana.  The most recent cases relate to surgeries performed in mid-January 2019.  The U.S. Mission to Mexico has advised US. government employees to avoid seeking medical treatment at the Grand View Hospital until further notice.  Refer to CDC’s Travelers’ Health Alert on Drug-Resistant Infections in Mexico for additional information and future updates.

Isaac Zuñiga, the First Corneal Transplant Surgeon in La Paz By Susan Fogel Two years into medical school, Isaac Mauricio Zuniga Gonzalez wanted to run away and join the circus. Cirque de Soleil to be exact! “I became intrigued with trapeze artists. I took some classes and in six months I was an instructor,” says Zuniga. He did not have the floor techniques, but he knew where he had to go to learn all the skills required to be a professional trapezist-Canada. “I decided to sell the car my parents had given me when I entered medical school.” His dreams of running away with the circus were dashed when his mother reminded him that the car was actually hers and on loan to him while he completed his education. If he persisted in this foolishness of the circus, he was going to have to do it without the car! Zuniga did complete medical school at Universidad Nacional Autonomia de Mexico (UNAM) in Mexico City. He and his wife, Elizabeth “Karla” Jimenez, also an MD visited La Paz during their internships in Ensenada. They made a pact to start their married life and careers in La Paz. Jimenez found a job first at Centros de Rehabilitación e InclusiónInfantilTeletón(CRIT). Zuniga,an eye surgeon with a specialty in corneal transplants, knocked on doors in La Paz. He started at ISSTE, where he was offered a pay of $300 pesos for an 8-hour day’s work. At a low point in his job search, he considered selling hotdogs on the street. He would surely make more than 300 pesos for a day’s work! Through persistence and good timing, he landed a position with IMSS as a relief ophthalmologist, and that job became permanent. But he wanted more. He wanted to perform corneal transplants. He wanted to treat people with corneal eye diseases. People that would have to otherwise travel to Tijuana, Mexico City, or the United States for treatment. Continuing to knock on doors and look for possible partnerships or anything that would lead to his life’s calling. Persistence, timing, and luck intervened. His knock was heard, and the door opened by Miguel Fausto Lechuga, an ophthalmologist with offices at Fide Paz hospital. Lechuga is a retinal specialist. But he had patients, as did other eye doctors with diseases of the cornea. It took a year to obtain the permits and certifications for Zuniga to be allowed to perform corneal transplants in our state. And,with the support of Lechuga that helped him start a private practice in La Paz. He performed his first transplant in La Paz on March 20, 2015. On April 27, 2017, Zuniga performed a corneal transplant on me. This was my third in 35 years. The other two surgeons are renowned in California and Mexico. Dr. Zuniga impressed me with his intelligence, competence, and confidence. Look at this picture of my eye before and after the transplant. I went from being blind in my right eye to 20:30 vision overnight! I was involved in the process of choosing the cornea. Zuniga showed me the internet site where the available corneas are listed. The medical details of the donor (except their names), were listed. Not only were those details listed, most importantly, the condition and cell count of the cornea -crucial details- were also on the site. My cornea came from a 42-year-old Native American man . Both Drs Zuniga and Lechuga call my new cornea “beautiful”.Lechuga said that the surgical procedure under Zuniga was like a “finely tuned machine”. My formerly blind eye is now corrected to 20:15. A Little Piece of Plastic, The Miracle of the Scleral Contact Lens By Susan Fogel Many years ago, I was hit in the head by a surfboard. I was dizzy and had headaches for days. My parents brought me to the doctor who could find nothing wrong. I was sent from specialist to specialist. One of whom was an ophthalmologist. He discovered that I had keratoconus. A disease of the cornea that caused it to lose its smooth conical shape, and eventually cloud over. In 1968, the only treatment was a corneal transplant. BUT, warned the doctor, the procedure was not perfected, and he did not recommend it.Over the years, my cornea deteriorated. In 1982, I had my first transplant. It required several-days stay in the hospital. There were 46 stitches and I had to wear a thin plastic lens to keep the stitches from irritating my eyelid. When the stitches were finally removed, I was fitted withstandard semi-rigid gas permeable lens(GP). In the doctor’s office, my vision tested at 20:15. However, the only time that the lens felt good was when it was hanging from my eyelash. I gave up on wearing GP lenses. But scleral lenses are bigger and deeper than GP lenses and they are designed for people with corneal issues. INSERT photo of lenses. They help combat dry eye and will dramatically improve vision. In fact, my eyes feel better with the lenses in than when they are out! Since 1982, I have had two more corneal transplants. The one before the latest was done in Tijuana. That cornea clouded over very quickly, and I just gave up on being able to see out of my right eye. My sight had deteriorated to the point where I decided not to drive anymore. I was afraid I would hurt someone. Dr. Miguel Fausto Lechuga was my ophthalmologist here in La Paz. He has been patient and kind and tried hard to keep my eye healthy. He is a retinal specialist, not a corneal specialist.He had been gently pushing me to go back to Tijuana for another transplant. I was resisting.Then he introduced me to Dr. Isaac Zuniga here in La Paz. And on April 27, 2017, I had another transplant. I also met a woman on an online sewing site(of all places!) that had just been prescribed scleral contact lenses for her keratoconus.The hunt was on for me to find those lenses. It was frustrating, and those that I could find were extremely expensive. After my recent surgery in La Paz, I met Dr. Cesar Enoc, an optometrist in Mexico City with a calling. He wants to help people with corneal issue to see better and to live without pain. Through an association with Felipe Arroyave, Cesar has become a representative for scleral lenses in Mexico.Felipe says that “…a little piece of plastic can change people’s lives…” He goes on to say that we have been given wonderful senses, and sight is the window on the world.This little piece of plastic brings comfort, color, and clarity. And in the case of diseases like Steve Johnson Syndrome, protects the cornea and eyelid as it improves vision.Felipe works mainly in Africa and South America, bringing relief to people that have corneal diseases. Cesar was trained at the UNAM School of Optometry and attended on a full scholarship. He visits and trains optometrists throughout Mexico in the fitting and use and care of scleral lenses.When he became interested in treating diseases of the cornea, he searched the world for a source of scleral lenses. He contacted a company in Boston. They were not receptive to Cesar and just wanted him to refer patients to them. The lenses provided by this company cost $7,500 US, added to that would be the travel expenses. This is far beyond the reach of most Mexicans.Eventually, Cesar hooked up with Felipe, who gave him his first fitting set of lenses. He has been trained and has also developed his own design of scleral lenses. Lenses manufactured by a lab for Cesar in the US are $750.00 per pair, that is onetenth of the cost of the other company! Cesar says: “We are dedicated to improving the quality of life of all people with corneal diseases. We will donate the little pieces of plastic, if we have to.”Once a religious missionary, and now a scientific missionary, bringing sight and relief from eye pain to others, Cesar says, “I changed my life by changing the lives of others.” Cesar visits La Paz on a regular basis and uses Dr. Zuniga’s office to see patients.Cesar will be visiting La Paz in January. Hi,
I had a great experience with Dr Zúñiga 2 months ago. I wanted to have a LASIK-surgery with him in Mexico City, but after checking my eyes in La Paz he found a tiny beginning glaucoma and he recommended me against the surgery. I totally trust him and his professional opinion. He was very knowledgeable and also took lots of time to explain everything patiently in English to my boyfriend who doesn't speak Spanish.

Ambulance fees: Persons who are transported to the hospital by ambulance are requested to pay for the service. This is a completely volunteer service. The ambulance crew spends quite a bit of time fund raising for gasoline and maintenance of the ambulances. Recommended fee is 1000 pesos per trip.
